Required Forms
Please print and complete all forms in this section if you have not already turned them in with 2011-2012 registration paperwork:
Transportation Agreement - will also need to bring a copy of your Driver's License and Insurance Declarations Page. NO COPY MACHINE will be available this year - copies should be made in advance and attached to form. Return to the PTF Table.
Parent Volunteer Application - all parents who will be spending any time at the school with kids or helping with Work-A-Thon need to complete this form. Return to the PTF Table.
Volunteer Opportunities Form - all parents should complete this form to indicate what areas they want to serve in this coming school year as they work towards completing their 10 service hours. Return to the PTF Table.
Medical Release Form - Return to the campus Nurse. (Dallas Road - Judy Davis, Worth Street - Denise Allen)
Elementary Dismissal Authorization Form - all Elementary parents should complete this form to designate who may pick up their child - if they want them to be dismissed to other adults/parents.
Student Driver Form - All students who drive a car to school need to complete this form. Return completed form at Orientation.
High School Parking & Traffic Map - All High School parents need this map to know what traffic flow will be at the Dallas Road Campus.

Dollar Days Signup - By participating in the Dollar Days program, your child will be allowed to wear jeans/shorts with a Faith t-shirt approximately twice a month. Cost is $15.
Medication Administration Permission - Complete this form to turn in to the campus nurses if your child will need medication during the year.
Fall Precepts Bible Studies - Please read the attached flyer for information about the 6 different Precept Bible Study classes held at Faith this year. Participants: men, women, students in Grades 3-5, Jr High students. Visit the Precepts table at Orientation to actually sign up.
Home Economics Class Registration (grades 3-12) - Helen Buckner, a Faith teacher, is offering a beginning and intermediate Home Economics class during the Fall Semester. Click this document to get details and the registration form.
PTF Yard Sign Order Form - Show your school spirit by displaying a Grapevine Faith Yard Sign in your front yard. Click this link for the registration form. Return completed form with payment to the PTF Table.
